Job Details

The R.N. II is the basic level of professional nursing practice. She/he identifies and

implements nursing interventions that have less predictable outcomes, and evaluates the results of these interventions for a given patient population. A nurse at this level may be required to assume leadership responsibility.


JOB RELATIONSHIPS:

A. REPORTS TO: Director, Assistant Director, Program Coordinator, or designee.

B. SUPERVISES: LPN's, LPNA's, Nursing Assistants, Patient Care Associates, and other ancillary staff

C. INTERRELATIONSHIPS WITH: All nursing staff, all ancillary and clerical departments' staff, physicians, patients and families.


HOURS OF WORK: Variable - rotating 8/10 and/or 12 hour shifts.
DRESS CODE: Per unit dress code.
ESSENTIAL REQUIREMENTS OF WORK (Minimum qualifications necessary to function at full

productivity).

A. EDUCATION AND TRAINING: Graduate of an accredited school of nursing. Current licensure in the State of Minnesota.

B. EXPERIENCE: Maintains RN I criteria and meets RN II criteria.

C. JOB KNOWLEDGE (Specific): Working knowledge of professional nursing theory and practice.

Ability to communicate effectively orally and in writing. Ability to establish and maintain effective working relationships with patients, employees, public, and other providers.

A knowledge of growth and development; specifically an understanding of the cognitive, physical, emotional and chronological maturation process. An understanding of the range of treatment needed by patients of all ages in relation to the growth and development processes to include neonate, child, adolescent, adult and geriatric patients.

pumps/exercisers/suctions/respiratory/heating and cooling/vital sign measurement equipment, computer, bar code scanners, copy machine, medication dispensing machines, beds, carts, lifting devices, wheelchairs, telecommunication devices.

A. JUDGEMENT AND INITIATIVE: Requires continuous mental effort for decision making and problem solving in a variety of patient care situations, many of which are complex and/or stressful in nature.

B. INDEPENDENT ACTION: Needs to perform at independent level with minimal direction.

C. EFFECT OF ERROR: High liability for nurse and hospital for patient care errors - error

unacceptable.

D. CONTACT WITH OTHERS: Frequent multiple contact with other care providers and ancillary support staff, patients/families.

TASKS: 1. Assessment/Diagnosis

A, Establishes relationship with patient and family and introduces self and explains role. B. Applies effective interviewing skills to elicit information from the patient and/or family/significant other that is necessary to plan, implement, and evaluate nursing care. C. E. Identifies and interprets abnormal diagnostic data and incorporates it in the written assessment.

F. Identifies verbal and nonverbal communication problems of patient and/or family.

G. Integrates assessment data and identifies all appropriate priority nursing diagnoses. H. Identifies implications of patient age in assessment of needs and identification of nursing diagnoses. I. Assesses the needs of a specific patient population by:

1) Establishing appropriate prioritization of their patient assignment.

2) Collaborating with physician, care managers, unit coordinator and other

members of the health team as appropriate.

2. Planning

a. Identifies expected outcomes for planned nursing interventions.

b. Interprets data obtained by self & others and incorporates this in the nursing care plan.

c. Participates in the organization and implementation of formal teaching plans. d. Identifies nursing interventions appropriate to patients age.

e. Collaborates with patient/family, care manager, social worker, unit coordinator

and others to coordinate/plan patient care, patient discharge and documentation of same.

f. Establishes priorities for patient care plan.

g. Establishes thorough patient care plan within 8 hours of admission.

h. Communicates and directs the Patient Care Plan to other members of the multi-

disciplinary team.

3. Implementation

a. Establishes priorities and implements nursing actions on the patient care plan by either direct care or delegation of responsibility.

b. Communicates accurate information about the nursing care plan to the patient

and/or family.

c. Documents patient care problems utilizing assessment data.

d. Demonstrates the ability to utilize the requisite technology relevant to the specific clinical area (see unit skills checklist).

e. Implements and/or assists others to implement the teaching plan.

f. Recognizes the need for post hospitalization care and follow-up.

g. Works with care manager, social worker and others, and makes recommendations

for referrals, etc.

4. Evaluation

a. Interprets initial assessments and revises these assessments based on patient's response.

b. Utilizes data obtained from patient, family, and/or significant others to re-evaluate

patient care.

c. Collaborates with nursing colleagues and multidisciplinary team members regarding effectiveness of the Patient Care Plan for each patient.

d. Evaluates and documents to what extent nursing goals have been met for a select

group of patients.

Registered Nurse II 8242-43 SPECIFIC JOB DEMANDS:

• Sitting for report, documentation and at times when working with patient.

• Repeat use of hands for cares- i.e. vital signs, medication administration- either oral or IV - suctioning, patient's education.

• Moving around the floor and specifically between assigned patients and specifically to follow through with patient care.

• Forward/upward/downward reaching when caring for a particular patient and obtaining supplies.

• Assist with patient ambulation to build tolerance.

• Transfers in and out of bed, chairs, wheelchair and other means of moving a patient.

• CPR and the related activities. An ability to be change positions to do chest compressions airway ventilation, etc.

• Patient cares done with assistance or alone depending on needs of patients on any shift. Lifting and moving patients, CPM machines, and positioning a patient's extremity in the unit.

• Over the head reaching for IV bag hanging, lifting bags of solutions of various weights.

• Visual acuity is needed for doing activities related to medication administration.
