We understand the personal impact caused by significant life and health changes and are committed to care that makes our residents feel reassured and fulfilled.

A professional staff provides focused rehabilitation services and our dedicated memory care and palliative care teams attend to individuals with special needs around the clock.

Job Duties/Responsibilities

• Assists residents with personal cares including bathing, dressing, grooming and toileting as needed and in accordance with care assignment ensuring resident are clean, neat and well groomed.
• Provides all cares in accordance with resident desires and in a manner that is respectful and dignified.
• Follows all shift routines, policy and procedures regarding accurate performance of vital signs, weights, etc.
• Serves food in accordance with dietary card, assists residents with eating and accurately records food and fluid intake. Reports any chewing or swallowing difficulty to the nurse.
• Promptly reports any observed or reported changes in residents’ functioning, mental status, behaviors, abilities, concerns or changes.
• Provides all care and perform all duties in accordance with safety and infection control standards and reports all issues/concerns (e.g. equipment malfunction) to charge nurse.
• Documents all cares and data according to facility protocol.
• Requires a CNA certification
