Description

The Community Health Social Worker supports the mission and goals of Lakewood Health System through personal and professional conduct including concern demonstrated for all people served, regardless of age, race diagnosis or economic background.  They are an active member of the Lakewood Health System team by contribution of ideas, suggestions, and regular work attendance, and punctuality, adherence to Lakewood Health System rules, policies, behavioral standards and patient confidentiality.  Abides by and supports Lakewood Health System’s Employee Standards of Behavioral Excellence.

  • Serves as social worker for clients/families in the work setting of the Lakewood Food Farmacy.
  • Completes social service/psychosocial needs assessment of client/family in partnership with Community Health Coordinator.  Primarily serves complex families and lead IHP case management processes.
  • Identifies psychosocial problems and include them in EPIC documentation, along with intervention and goals.
  • Completes necessary follow-up, as needed, with patients and document in the Community Health progress notes.
  • Attend Interdisciplinary team meetings and provide pertinent information as needed to the interdisciplinary teams across the system.
  • Acts as a referral source to community health team and provide community resource information across the system.
  • Serves as a liaison to clinic / hospital departments and Food Farmacy operations.
  • Assists in the coordination, participation and representation of the Community Health team within internal and external collaborations and community events.

Qualifications

  1. Must have at least a bachelor’s degree from a social work curriculum accredited by the Council on Social Work Education
  2. Must be a Licensed Social Worker for the Minnesota Board of Social Work
  3. Demonstrates the ability to function with an interdisciplinary team to establish cooperative relations with colleagues and to provide support to the clients served.
