Answers patient inquiries over the telephone, assessing the severity of the patient's symptoms and medical needs according to triage algorithms. Guides the patient to the appropriate level of care; schedules patient appointments over the phone with appropriate healthcare providers/facilities/physicians. Documents information provided to patients during triage in health record systems; records patients health conditions for follow-up patient care. Maintains and develops triage protocols; ensures patients' healthcare information is updated on time. Role does answer inquiries through multiple variations of technology, depending on department. Educate patients based on calls that come in.
Builds a rapport with the patient and explains the condition and course of action effectively. Coordination and management of care for patients, including with complex needs. Delivers written and oral communication responds to questions and concerns, and produce specific outcomes and impact. Explains treatments and procedures to patients suffering less serious illnesses and injuries. Effectively delegates and supervises based on ones state practice act and an understanding of the concepts of responsibility, authority, and accountability. Performing age related patient assessments and nursing interventions. Performs triage, sorting patients needing urgent care from those with less severe injuries. Provides direct service to internal or external customers. Responsible for promoting a safe environment for patients, families, staff, residents, and visitors and working collaboratively with the multi-disciplinary health care team to provide the best outcomes for patients.
Four 10 hour shifts per week. Day off will vary, can be flexible on day off. Educates patients on diagnosis, treatment, and chemotherapy regimens. Manages oral chemotherapy. Assists with in clinic procedures. Assists with care coordination for patients.
Currently holds an unencumbered RN license with the State Board of Nursing where the practice of nursing is occurring and/or possess multistate licensure if in a Nurse Licensure Compact (NLC) state. Nurses performing nursing practice over the telephone require licensure in the states where the patients being served reside. Within first 30 days of employment, all requirements to obtain license needed for role is completed. Obtains and subsequently maintains required department specific competencies and certifications. Obtains and subsequently maintains required department specific competencies and certifications. Graduate from an accredited nursing program preferred, including, but not limited to, American Association of Colleges of Nursing (AACN), Accreditation Commission for Education in Nursing (ACEN), and National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA). Bachelor’s Degree in nursing preferred. One year of healthcare experience preferred.
