Oncology Case Manager

Lakewood Health System has an opening for a self-motivated, dedicated RN Oncology Case Manager within our Staples Clinic.

The RN Case Manager will work directly with our Oncology team and patients in the clinic setting.  The Oncology Case Manager is expected to complete the rooming of patients, assist with procedures, provide  Oncology specific education directed by the providers, assist with medication management and questions as directed, in-basket management, pre/post visit planning with scheduling, medication monitoring of labs, infusion tracking, complete patient calls, review and route medication refills and follow up with the Oncology patients.

This is a fulltime position working 64-80 hours every 2 weeks; Monday through Friday between the hours of 8:30am-5:00pm.

Qualifications

1. Current MN RN Licensure

2. BLS Health Care Provider certification

3. Case Management and EPIC experience is preferred.

4. Oncology Experience Required

5. Ability to work independently; project a positive and professional image and excellent interpersonal communication skills.
