Job Details

To provide medical social work services to patients/families in order to meet psychosocial needs of patients and families. Comprehensive social work services are provided, including patient/family education support, and resources information and referral for patients and families. To participate in professional, departmental and hospital activities. Is a member of the interdisciplinary team. Facilitates bereavement support groups and coordinates bereavement closures. Coordinates appropriate community referral for patients and bereaved. Assists with the coordination of children's grief camps.

MSW provides professional clinical social work with individuals, families, and groups through the implementation of clinical modalities, i.e. individual, group and family therapy. Additional responsibilities include clinical supervision of graduate social work interns and required LSW employees.


JOB RELATIONSHIPS:

A. REPORTS TO: Hospice Program Social Work Coordinator

B. SUPERVISES: None

C. INTERRELATIONSHIPS WITH: Physicians, various hospital employees, Hospice team members, outside agencies.


HOURS OF WORK: Variable, Monday through Friday, variable part-time as needed.


DRESS CODE: Street clothes in good taste.


ESSENTIAL REQUIREMENTS OF WORK (Minimum qualifications necessary to function at full productivity).

A. EDUCATION AND TRAINING: MSW preferred, or baccalaureate degree in social work from an institution accredited by the Council on Social Work Education; or a baccalaureate degree in psychology, sociology, or other field related to social work and is supervised by an MSW.

B. EXPERIENCE: At least one year in medical social services.

C. JOB KNOWLEDGE (Specific): Knowledge of the psychosocial needs of the dying patient and their families, knowledge of the grief process and knowledge of community resources, group dynamics, financial resources. Requires excellent verbal and written communication skills. Knowledge of support group facilitation.

Public speaking is required for community presentations. Knowledge of vulnerable adult and abuse regulations.

A knowledge of growth and development; specifically an understanding of the cognitive, physical, emotional and chronological maturation process. An understanding of the range of treatment needed by patients of all ages in relation to the growth and development processes to include neonate, child, adolescent, adult and geriatric patients.

CLIENTELE DIRECTLY AFFECTED BY JOB: Hospice patients and families, hospital employees, outside agencies.


PHYSICAL DEMANDS OF THE JOB: Walking, Standing, Lifting, Driving


PHYSICAL ENVIRONMENT OF THE JOB: Well lighted, ventilated, even temperature environment. Work outside of the facility (ie: patient's homes and nursing homes) is required. Frequent driving in normal to adverse conditions.


EQUIPMENT OR MACHINES USED ON THE JOB: Telephone, cell phones, computers, fax copier.


MENTAL DEMANDS OF THE JOB:

A. JUDGEMENT AND INITIATIVE: Must exercise sound judgment working with patients and staff. Needs to follow strict confidentiality standards.

B. INDEPENDENT ACTION: Works independently and must be able to work within other people's deadlines as well as for the sake of patient care. Must be flexible and be able to reevaluate priorities as situations arise.

C. EFFECT OF ERROR: Error could cause very serious consequences in some cases, negligible in others.

D. CONTACT WITH OTHERS: Must be able to work well with hospital employees, both in and out of the Social Service Department, Hospice team members, as well as patients, families, physicians, Nursing home staff and representatives of other agencies.


PRINCIPAL JOB RESPONSIBILITIES, TASKS, AND AUTHORITIES:

A. RESPONSIBILITY: Provide medical social work services with patients and families within the Hospice program.

TASKS:

Social Worker-Hospice 3310

A. MSW/LSW provides direct counseling/therapy to patients/families as needed.

B. MSW/LSW consults with appropriate mental health professionals when concerns about patient's clinical diagnosis arise.

C. Assists in the assessment of patient/family needs. D. Serve as a member of the interdisciplinary team. E. Attend Hospice team meetings. F. Coordinate, with other team members, the use of community resources and

refer as appropriate. G. Participate in the discharge planning of hospitalized Hospice patients. H. Plan, organize and facilitate Hospice bereavement support groups. I. Assist in developing/providing support services for staff. J. Assist in planning and provide periodic professional and non-professional

training programs. K. Interpret philosophy, goals, objective, policies and procedures of the program

to patients/families, staff and the public. L. Attend Hospice related meetings, workshops, and conventions.

AUTHORITIES: Has the authority to carry out the above tasks.

A. May need to attend Community Advisory Meetings. B. Attend departmental staff meetings. C. Participate in hospital committees as assigned. D. Participate in continuing education opportunities to increase knowledge and

enhance skills. E. To perform other tasks/functions as delegated by the department director in

order to meet patient and department needs. F. To present educational/inservice sessions to area agencies, nursing homes

and the community. G. Facilitate bereavement support groups.

AUTHORITIES: Has the authority to carry out the above tasks.

C. RESPONSIBILITY: Quality Assessment Performance Improvement

Tasks:

1. Demonstrates understanding of the QAPI philosophy. 2. Participates in QAPI training activities or QAPI teams if appropriate. 3. Demonstrates sensitivity to customers and their needs. 4. Interacts appropriately with internal customers, i.e. coworkers within

department, staff across departments. 5 Interacts appropriately with external customers, i.e. patients, families,

medical staff, vendors. 6. Role models positive behaviors.

D. RESPONSIBILITY: Safety - Maintain and promote a safe environment for all patients, visitors and staff.

Tasks:

1. Consistently follow all policies, practices and work rules. 2. Do not use shortcuts or "work-arounds" that may reduce safety or

increase risk. 3. Stay alert, act responsibly and use common sense to reduce risks. 4. Report "actual events" and "good catches" as soon as possible. 5. Create a safe environment by eliminating hazards and identifying and

reporting unsafe systems. 6. Complete all mandatory safety education, attend safety sessions, review and

understand the Safety Program (available on RiceNet). Seek answers to questions you have about the Safety Program.
