Job Details
The ideal candidate will have:
1. Master's Degree in Social Work (preferred) and licensed in the State of Minnesota
2. Advanced Directive Trained/Certified, with experience in leading committees
3. Excellent oral and written communications skills
4. Knowledge of community resources
5. Ability to deal with end of life process,
6. Knowledge in bereavement and Care Management.
7. Develop a rapport with client and family.
8. Demonstrates problem solving and decision making skills
