JOB SUMMARY

The Registered Nurse (RN) is responsible for utilizing the nursing process (assessment, diagnosis, outcomes/planning, implementation and evaluation) to provide professional nursing services to patients at home. The nursing process includes the documentation of human responses to actual or potential health problems in the home health environment. This practice includes dependent, interdependent and independent functions. Collaborates patient and family, other inter-professional colleagues, such as physicians, to plan, implement and evaluate care. Demonstrates competency and practices within the full scope of nursing expertise/knowledge and utilizes appropriate age and population specific standards as designated in their assigned setting. Cares for patients in all phases of preventative care, health maintenance, diagnosis, treatment, and follow-up for patients in the home setting. Responsible to implement the nursing process within Sanford, the coordination and continuum of care, patient assessment, patient education, triage, and various other nursing interventions. Functions within the scopes and standards of nursing practice as outlined in the Nurse Practice Act and Administrative Rules in state of practice and licensure. The Sanford Professional Nursing Practice recognizes the Scope and Standards of Practice and the Code of Ethics for Nurses with Interpretive Statements as published by the American Nurses Association as the foundation of nursing care delivery and professional conduct.

POSITION RESPONSIBILITIES

Adheres to standard processes while working with a specific function of home based healthcare services. Applies home healthcare operating principles and practices within own position. Contributes to organization and industry best practices for documenting information. Coordination and management of care for patients, including with complex needs. Delivers primary healthcare services in a home based setting. Delivers written and oral communications that engage audience participants, respond to their questions and concerns, and produce specific outcomes and impact. Effectively delegates and supervises based on ones state practice act and an understanding of the concepts of responsibility, authority, and accountability. Establishes and maintains productive working relationships within and outside of own area. Guides others in implementing given health and safety guidelines for home based healthcare services. Performing age related patient assessments and nursing interventions.

DEPARTMENT DETAILS

This is a pediatric position working extended hours in patients homes in Grand Forks. Start and stop times typically 730 am-530 pm. Shifts available Monday through Friday, some PM and weekends available as needed. This is a PRN position. It requires a MN and ND license, we reimburse for the secondary license.

QUALIFICATIONS

Currently holds an unencumbered RN license with the State Board of Nursing where the practice of nursing is occurring and/or possess multistate licensure if in a Nurse Licensure Compact (NLC) state. Obtains and subsequently maintains required department specific competencies and certifications. Valid driver's license and proof of insurance required. Basic Life Support (BLS) required. Certificate for Outcome and Assessment Information Set (OASIS) Specialist - Clinical (COS-C) preferred.
