JOB SUMMARY

Coordinate, negotiate, educate, procure services for, and manage the care of, complex patients to facilitate achievement of quality and cost-effective patient outcomes.
Knowledge of case management principles, professional practice standards, health and public policy, consumer health care issues and current trends in health care desired. Knowledge related to reimbursement, finance, allocation of resources, quality improvement, quality management and utilization management are desirable.

POSITION RESPONSIBILITIES

Explains treatments and procedures to patients suffering serious illnesses and injuries. Displays the critical characteristics of unrelenting candor, breadth of competence, and client empathy in all interactions. Demonstrates open, friendly, accepting, and supportive behaviors with team members. Responsible for promoting a safe environment for patients, families, staff, residents, and visitors and working collaboratively with the multi-disciplinary health care team to provide the best outcomes for patients. Identifies resources best suited to treat patients' conditions and contacts appropriate specialists. Identifies non-verbal emotional indicators in others, their negative impact on results, and pursues action to mitigate them for improved success. Helps patients and caregivers with language/literacy barriers to ensure comprehension of information. Guides patients in scheduling appointments and laboratory tests according to established processes and policies.

QUALIFICATIONS

Bachelor's degree required; nursing, social work, or counseling. RN preferred. Three years of experience and three years of management/leadership experience desired. Knowledge of case management principles, professional practice standards, health and public policy, consumer health care issues and current trends in health care desired. Knowledge related to reimbursement, finance, allocation of resources, quality improvement, quality management and utilization management are desirable. Currently holds an unencumbered RN license with the State Board of Nursing where the practice of nursing is occurring and/or possess multistate licensure if in a Nurse Licensure Compact (NLC) state. Or currently holds a Social Worker license (SWL) with the State Board of Social Work Examiners where the practice of social work is occurring. Obtains, within 36 months, and subsequently maintains required department specific competencies and certifications.
