Clinic Behavioral Health Home Intergrations Specialist (BHH)

Lakewood Health System has an opening for a Clinic Behavioral Health Home Integration Specialist (BHH) at our Staples Campus.

This is a part-time position working 20 hours per week, various days of the week based on program needs.

Looking for a LICSW or LMFT (Licensed Marriage and Family Therapist) to build and promote our new Behavioral Health Home. As an original leader in Health Care Homes, LHS is excited to be launching this program along-side our award winning Medical Home. Join our interdisciplinary team to collaborate with both medical and behavioral health providers.

Highlights of this role include:

• Working with members to complete assessments, goal setting and referrals
• Facilitate small groups
• Public promotion of the program
• Collaboration with medical and behavior health providers within and outside of LHS
• Multidisciplinary team
• Internal and Community Educational Events

Qualifications

1. Master’s Degree in Social Work with State License as LICSW. OR…
2. Licensed Marriage and Family Therapist
3. Previous Counseling and Behavioral Health Experience is required
4. Trained in a variety of treatment philosophies to respond to varying patient treatment needs.
5. The integration specialist has to comply with the Minnesota Statutes. Section 245.4871, subdivision 27, clauses (1)-(6) or Minnesota Statutes, section 245.462, subdivision 18, clauses (1)-(6)
