Candidate must be a North Dakota Licensed Social Worker

On-site childcare available from 5:30 AM - 11:00 PM seven days a week.

Free on-site wellness center open 24-hours a day.

Experience in health care- long-term care is preferred. Computer skills are helpful. Must have strong interpersonal skills and the ability to work in a changing environment. Must also have excellent organizational skills.

ESSENTIAL JOB FUNCTIONS:

Assist with the process of resident admissions, transfers, and discharges. Complete all necessary documentation, according to the federal, state, and local rules and regulations.

1. Participate in marketing, and is available to provide more detailed information on Eventide's continuum of care to prospective clients and families.
2. Provide information, counseling, referrals, orientation and education for staff, residents, families, and the community and is available as a resource to other providers and interested parties.
3. Act as a facility resource to provide intervention with difficult resident and family issues.
4. Coordinate and facilitate resident care conferences including initial, quarterly, change of condition, special needs, and discharge.
5. Assure Social Service charting and documentation is completed according to federal, state, local, and facility policies and procedures.
6. Assist in the planning of resident care services as they apply to the psychosocial needs of the residents and families.
7. In conjunction with resident care team, constantly assesses the appropriate placement of current residents on individual units.
8. Participate in meeting to further enhance the facilities social services program.
9. Assumes other responsibilities as requested by the Admissions Coordinator or DON.
10. Continually update knowledge base and skills by attending inservices, continuing education programs and staff meetings. Serve on committees as requested,
11. Update Social Service policies and procedures as needed.
