Job Details
The Vascular Access Resource Nurse collaborates with physicians and mid-levels; performs vascular access assessments on patients; provides patient care maintaining clinical skills; performs pain assessment; demonstrates ability to set up and use equipment required to place a PICC; provides comprehensive PICC care; functions as a resource; monitors patient outcome measurements; identifies, conducts follow-up and takes action on unsatisfactory outcomes, unusual finds, special needs and requests; participates in legal/risk occurrence tracking, follow-up and action; conducts variance analysis and manages achievement of consistent and/or expected clinical outcomes; collaborates with the Case Manager; coordinates communication with patient's allied health professionals and referring health care facilities; initiates professional growth and education opportunities for staff and students and acts as a liaison with Purchasing Department. Patients cared for include- infants, children, adolescent, adults, and geriatric.
Title and Description
Collaborates with physicians regarding the patient's vascular access needs. Performs vascular access assessments on patients assuring continuity of care and taking action on any requirements not met.
Conducts variance analysis and manages achievement of consistent and/or expected clinical outcomes.
Monitors patient outcome measurements. Identifies, conducts follow-up and takes action on unsatisfactory outcomes, unusual findings, special needs and requests
Coordinates communication with patient's allied health professionals and referring health care facilities, as needed. Collaborates with the Social Worker and/or Case Manager to facilitate discharge planning with vascular access device.
Provides comprehensive PICC care, including patient assessment, technical placement and appropriate follow-up care.
Participates in legal/risk occurrence tracking, follow-up and action in conjunction with members of the multidisciplinary team. Assures completion of documentation.
Acts as a liaison with the Purchasing Department regarding product selection and product change assuring quality, safety and cost containment.
Functions as a resource to staff and physicians regarding clinical and patient care issues.
Performs other duties as assigned or needed to meet the needs of the department/organization.
Demonstrates understanding and follows infection control policies and procedures according to standard operating procedure.
Maintains requirements for age specific competencies for the position and demonstrates the knowledge or principles of growth and developments and the skills necessary to provide patient/customer care/services.
Demonstrates knowledge and understanding of and compliance with:
•All pertinent safety, health and environmental policies, procedures and guidelines.
•Working safely, without causing harm or risk to self, others or property.
•The work environment safety procedures and prompt reporting of unsafe practices, procedures, accidents, injuries or other safety violations to the Leader.
•The use of all safety equipment and personal protective equipment.
