This is a full-time position working Monday-Friday from 8am-4:30pm.



Scope:
  • Responsible for centralized monitoring for home care clients with telemonitoring units.
  • Help enhance technology program and improve processes.
  • Assist with data collection and tracking for QAPI.

Key Responsibilities:
  • Responsible for centralized monitoring on HRS units. Telehealth Nurse will receive/monitor client text messages, phone calls, and video calls during business hours. Will follow up with clients as needed and contact skilled team as needed for alerts.
  • Ensure pathways are being followed per client’s plan of care. Responsible for educating staff on utilizing the HRS units.
  • Schedule video visits for clinicians and client, serving as the main point of contact for clients with the HRS units. Also, responsible for coordinating delivery and pick-up of the HRS units.
  • Work with Client Coordination Manager to help enhance technology program and improve processes.
  • Complete data collection and tracking for QAPI to be used for outcome analysis.

Education and Experience Required:
  • LPN License Required
  • Able to physically perform the duties required by the position.
  • Required to have the ability to read, write and speak English well enough to communicate with staff, residents and visitors.
