The Minnesota River Area Agency on Aging®, Inc. (MNRAAA) is looking to fill a Community Living Specialist position with the Senior LinkAge Line. Candidates must live in one of the following counties: Chippewa, Lac qui Parle, Yellow Medicine, Swift, Lincoln or Kandiyohi counties. Specialists assist private-pay nursing facility residents relocating from a nursing facility back to a community setting. In addition, Specialists also assist private pay consumers residing in community settings with developing a support plan to remain in their homes.

A minimum of a bachelor's degree or a master’s degree from an accredited college or university in registered nursing, social work, gerontology or related human services field is required. Experience in care coordination and case management is essential AND previous relevant employment experience at a long-term care facility, assisted living, hospital, home care or lead agency is required. 


Candidates must have experience with activities such as discharge planning, home and community-based service coordination or person-centered support planning. A valid State of Minnesota driver’s license, proof of insurance and reliable car is required. A degree in social work requires licensure or demonstrated progress/intent on securing licensure. A degree in nursing requires a current Minnesota Registered Nurse License.
