Social Worker

The Inpatient Social Worker screens and assesses psychosocial needs; identifies discharge planning/teaching needs; provides information and referrals to patients/families regarding resources and services; develops and coordinates discharge plan and arranges post-discharge services; coordinates/participates in formal and informal interdisciplinary staffings; assists with patient transfer; documents assessment, interventions and plan; acts as a resource person; provides support counseling and monitors and evaluates social work program effectiveness. Patients cared for include those in the following age groups: infancy, childhood, adolescent, adult and geriatric. Required: • Four (4) year degree in Social Work. • Licensure required by the North Dakota Board of Social Work Examiners. • One year of experience in a medical setting.
