Position description
The Palliative Medicine Licensed Practical Nurse (LPN) is a member of the interdisciplinary outpatient team providing comprehensive care of adult patients and families with various chronic and serious illnesses. The LPN is accountable for the delivery of direct patient care including medication reconciliation, focused assessment, re-enforcing education, and providing health information. The LPN collaborates with the care team for telehealth, prescription refills, and patient follow-up as appropriate for licensure. Seeking a LPN who is comfortable with ambiguity, is flexible, and enthusiastic about being a partner in the development of the LPN role in this setting.

The licensed practical nurse (LPN) provides nursing care to assigned patients under the direct supervision of the registered nurse (RN) or other assigned supervisor and accepts delegation from the RN/assigned supervisor in meeting the needs of the patient/family. The LPN collects data through observation and communicates information to assist the RN and/or provider in patient assessment and care planning. The LPN participates as a member of the health care team and accepts delegation from the RN/assigned supervisor and provider in meeting needs of the patient/family. The LPN delivers care in accordance with the patient care plan, policies and procedures of the organization and principles of relationship-based care. The LPN possesses excellent communication skills; is skillful in mentoring and instructing; and may participate on committees or projects, including quality improvement projects.

Qualifications
Student or graduate of a school of practical nursing or passed LPN Boards after a defined period in a professional nursing program (e.g., some states allow RN students to take the LPN board exam after completing one to two years in the RN program). If graduation did not occur within the last two years, one year of LPN experience working in an applicable setting is required.

Additional qualifications
Excellent communication skills (verbal and written) required. Computer skills required, prior experience with electronic medical record systems preferred. Seeking an individual with strong leadership, critical thinking, and interpersonal skills. Must be able to adapt to change, be comfortable with ambiguity, and willing to be flexible. Must obtain prior to start date:

Current LPN license by applicable state requirements.

Midwest: Basic Cardiac Life Support for healthcare providers- BCLS - certificate of course completion required and must not expire within 3 months after start date. A scanned copy of the certificate of completion or transcripts, including your full name, must be attached to your profile to be considered.

???Red Cross (Title: CPR/AED for Professional Rescuers and Health Care Providers or BLS for Healthcare Providers)

Compensation Detail
Education, experience and tenure may be considered along with internal equity when job offers are extended. The minimum pay rate is $19.88 per hour.
