Scope:
  • The Community Liaison in the West Region will serve in a consultant role, acting as an advocate for Knute Nelson Home Care & Hospice services to hospitals, clinicals, senior living communities, and community partners and organizations.
  • Effectively manage relationships with our partners to ensure referral goals are met.
  • Responsible for growing market share and geographic territory for Knute Nelson services.
  • Success will be measured by total monthly referrals, average daily census, and other measurements determined by the Director of Business Development.

Key Responsibilities:
  • Account Management - Act as the point of contact for their referral sources and community partners. Build long-lasting, mutually beneficial relationships by always striving to find the solutions which best fit the individual needs of their organization and patients. This position is also responsible for leading the identification and resolution of complaints to ensure client satisfaction and identifying industry/competition trends.
  • Research and Development - Responsible for researching new opportunities for growth and preparing and implementing sales plans. Responsible for developing, growing and maintaining documentation in the organization’s Consumer Relationship Management tool including effective documentation of accounts, contacts and events associated with assigned territory. Responsible for developing, increasing and maintaining market share and relationships in a defined geographic area including hospitals, clinics, skilled nursing facilities, assisted living facilities, adult family homes and the medical community. Responsible for organizing and planning that all marketing supplies, sales collateral, tools and equipment are prepared and organized in preparation for events or presentations.
  • Community Support - Responsible for managing and facilitating Knute in the Community for the communities within the assigned territory. This may include participation in community events, service organizations, etc. to represent appropriate Knute Nelson lines of service.
  • Compliance and Education - Understand healthcare operations, legal guidelines, competitive analysis, and market place trends. Provide in-services and professional presentations for referral sources, community groups and other organizations, regarding Knute Nelson’s services, programs and outcomes.
  • Customer Service - Support a territory of branch offices and relationships among each with internal and external customers. Serve as a liaison between Knute Nelson and clinical/medical staff to ensure smooth referral process, high customer satisfaction and continuity of care.
  • Communication - Act as a consultant and educator for patients, families, communities and providers about a variety of Knute Nelson’s services. Promotes effective relationships and communication with all staff. Assist in updating and improving education and marketing materials to ensuring it aligns with Knute Nelson branding standards.Demonstrate ability to communicate clearly and effectively, make effective verbal presentations and write effective reports.
  • Other Duties as Assigned - The duties and responsibilities listed above are representative of the nature and level of work assigned and are not necessarily all inclusive.

Education and Experience Required:
  • Associate’s degree required. Bachelor’s degree preferred.
  • Preferred experience/responsibility in health-related field.
  • Daily travel is required.
  • Strong computer skills specific to Microsoft outlook programs including Word, PowerPoint, Outlook, etc.
  • Must be organized and able to prioritize daily activities based on overall impact with little supervision.
  • Strong technology skills specific to entering computer data such as a consumer relationship management tool or something alike.
  • Must complete and pass a state required background study.
  • Able to physically perform the duties required by the position.
  • Required to have the ability to read, write and speak English well enough to communicate with staff, residents and visitors.
